Free Sweeps Coins can be requested by mail using the Alternative Method of Entry described in Section 14 of the Official Sweepstakes Rules, including the handwritten request card, the Player ID, and the envelope requirements.
Each request is limited, and each request must be received within the period, and postmarked as, stated in the Official Sweepstakes Rules. Requests that do not conform will not be processed.
If you mailed an AMOE request and believe it was not credited correctly, download and complete the AMOE Claim Form below, sign it, and mail it to the address shown on this page.
Include the website name in the Website / Brand field, your account username or Player ID, the date you mailed your request, and any reference or mailing records you have. Keep a copy of the completed form and your mailing records.
Claims must be completed legibly, signed, and sent by mail. Claims are not accepted through the website, and no Sweeps Coins are credited automatically.
Every claim is subject to verification of your identity, your account, and your original AMOE request under the applicable Official Sweepstakes Rules. Submitting a claim does not guarantee approval or credit.
Incomplete, inaccurate, duplicate, ineligible, or unverifiable claims may be denied.
